Job Details:

• Pay: $2480 per week (to be updated)

• Duration of assignment: 13 weeks

• Positions Open: 1

• Shift: Variable

• 80% outpatient and 20% inpatient

• Plans and implements specific treatment programs for patients according to the principles and practices of Physical Therapy.

• Evaluate patients and establish a written plan of treatment with short, and long term goals.

• Keep appropriate documentation records, ie: daily logs, progress notes, discharge notes, etc.

• Maintain open lines of communication within department, between departments and with the physicians.

Requirements:

• Active Unrestricted MT License

• Active BLS certification through American Heart Association

• Must have at least 1 year of  experience

• Must be able to provide 1 supervisory reference
